receive appropriate medical
treatment if required.

treatment you may require to be
explained clearly to you.
- Comment on
the care you receive, if you are
not happy you should always
speak to someone so that the
problem can be out right.
- Make a
complaint, if you feel the need
to make a complaint please
follow the practice complaints
procedure outlined in the
practice leaflet.
- Access to
your medical records. Please see
the section headed access to
medical records.
- For any
information that we hold on you
to be treated with
confidentiality.
It is your
responsibility to:
- Treat the
Surgery staff with politeness
and courtesy.
- Understand
that some patient’s may take
longer than others, so we ask
that you be prepared to wait.
- Listen
carefully to the advice on your
treatment and medication.
- Do what you
can to look after your own
health and follow advice on a
healthy lifestyle.
- Attend the
surgery for any health reviews
when requested to do so.
- Ensure that
you have sufficient supplies of
medication at all times.
- To keep
appointments, if you cannot make
an appointment please let the
surgery know as soon as
possible.
